Dismiss branch approval

veryfront__dismiss_branch_approval

The approval is soft-dismissed: the row is kept with dismissed_at, dismissed_by and dismissed_reason for audit, and it no longer counts toward merge eligibility. The legacy deleted: true flag means removal from the active approval set, not physical deletion. Requires project.files.write; API keys also require delete scope.
